Notes in Clarity can support care coordination and agency operations and case management. This article provides instructions and best practices for the use of Clarity Notes


Notes

Clarity Notes should be linked to Programs in the client file. To add a note, navigate to the relevant program and click ADD NOTE.

Add notes THROUGH client programs, not from the NOTES menu at the top of the record.

Notes for all programs can be reviewed in the main Notes menu item of the client file. The program creating the note is shown by hovering over the note type.

A program linkage icon is shown for linked notes. Hover the icon to view which Program created the note.

There are several note categories. The most common note categories and their uses are below:

Note Category Category Use Program Linkage
Case Note (General) Use this for general case notes
Vet CE Case Conference Use this for specific notes related to housing navigation for veteran case conferencing. CE Program, Veteran specific programs, i.e. SSVF, VASH.
CE Case Conference Use this for specific notes related to housing navigation. CE Program
Private Note Use this note category for Notes that will be marked as private using the private toggle at the bottom of the note page. Agency Programs only, private notes in CE Program will still be viewable by all users with CE access.

Note formatting can be defined by the agency. There are a few helpful formatting tools on the note worksheet. Numbers and bulleting can be used to make client goal lists or action lists. Use bold and italics to emphasize key details or phrases in the note.
