Reassessing with a New Type of VI-SPDAT

When a client is reassessed with a new VI-SPDAT type, they must be removed from the queue, and re-referred with their original referral date.

This step only needs to be completed when the type of VI-SPDAT changes VI-SPDAT Types: TAY, Individual, or Family VI-SPDAT


To Remove a Client From the Queue

Before removing the client from the queue, take note of the date of the initial referral to the queue, as the new referral will need to be backdated to match the original referral date.



Open the client profile → Select history → Edit the pending referral → Scroll down to “Remove From Queue” → Select “Reassessed” → Select Reassessment Type (Family, Single, TAY) → Save changes




After the client has been removed from the queue, the corresponding referral will change from “Pending” to having an end date


Re-Referring a Client to the Queue

After the referral has been removed, the new VI-SPDAT should be completed. The CM can now refer the client to the queue using the same workflow as the original process.


Return to the History tab to ensure there is a new pending referral → Edit referral → Change the date to match the initial referral date → Save Changes


Examples

●     In January, Alice completed an individual VI-SPDAT with her case manager. In April, Alice had a baby while still experiencing homelessness. In May, when meeting with her case manager, Alice completed a Family VI-SPDAT.

○    Because Alice was reassessed with a new type of VI-SPDAT, she will need to be removed from the queue, and re-referred.

●     Carl has been homeless for several months and completed a VI-SPDAT with his case manager. During this period of homelessness, Carl meets his partner Angie, who will now join his household. Because of the change to his household, Carl should complete a new Individual VI-SPDAT.

○      Carl had a change to his household, which signals that a new assessment should be completed to capture any changes. However, because there are no children involved, an individual VI-SPDAT will still be used. Carl does not need to be removed and referred to the queue again.

●     Coco is a 19 year old individual who is experiencing homelessness. She completed a TAY VI-SPDAT with her case manager. Coco begins a relationship with 23 year old Trevor and they have a child together. The three individuals are now a household, with Coco as the Head of Household.

○      Coco will need to complete a Family VI-SPDAT with her case manager. Her initial referral will need to be removed from the Permanent Housing Queue and she will need to be re-referred from the new VI-SPDAT.
