Veteran List Status Assessment
Overview
The Veteran List Status Assessment is used to track a Veteran’s current status on the By-Name List (BNL) and their progress toward permanent housing. This assessment supports:
1. Service Triage
2. Veteran Benchmark Reporting
3. VA One Team BNL Initiative
How to Complete the Assessment
Follow the steps below:
- Search for the client in HMIS.
- Go to the client’s Profile.
- Navigate to the Programs tab.
- Locate the client’s SSVF or CES* enrollment.
- Click the pencil icon to edit the enrollment.
- Click on the Assessments tab.
- Select or create the “Veteran List Status” assessment.
- Complete all required fields.
- Save.
*Released for SSVF programs ONLY first and then expanded to CES in March after CoC can approve expansion.
When to Complete This Assessment
The Veteran List Status Assessment must be completed every time there is a change in the Veteran’s status.
Complete a new assessment when:
- The Veteran’s housing situation changes (e.g., unsheltered to sheltered)
- The Veteran becomes permanently housed
- The Veteran becomes inactive or cannot be located
- The housing plan track changes
- The Veteran relocates outside SLO County
Do NOT complete a new assessment simply because you met with the Veteran again, if their status has not changed.
Data Fields and Definitions
Users may need to collect up to seven (7) data points.
1. Assessment Date
Definition: The date the assessment is being entered into HMIS.
- This is typically today’s date.
- This reflects when the information is recorded in the system.
2. Veteran List Status
Select the appropriate status from the dropdown:
Active Statuses
These indicate the Veteran is actively receiving or requesting services.

Active – Sheltered
Veteran is actively engaged in services and is currently staying in Emergency Shelter (ES) or Transitional Housing (TH).

Active – Unsheltered
Veteran is actively engaged in services and is currently living in a place not meant for habitation.

Active – Other
Veteran is actively engaged in services but is currently in an institutional setting (e.g., jail, hospital).
Inactive Statuses
These indicate the Veteran is no longer actively participating in services locally.

Inactive – Permanently Housed
Veteran has obtained permanent housing and no longer needs services.

Inactive – Unknown/Missing
Veteran cannot be located or contacted despite outreach attempts.

Inactive – Other
Veteran has moved out of SLO County and is no longer eligible for local services.
Data Quality Option

Data Not Collected
Contact with the Veteran occurred, but the status information was not obtained.
3. Veteran List Status Date
Definition: The date the status became effective OR the date you became aware of the status change.
This is not necessarily the assessment entry date.
Example: If you learn on March 10 that the Veteran moved into shelter on March 1, use March 1 if known. If the exact date is unknown, use the date you became aware.
4. Veteran PH Plan / Track
Select the permanent housing pathway the Veteran is expected to pursue:
- SSVF – RRH
- Other RRH
- HUD-VASH
- Other PSH
- Other PH
- Self-Resolve / No Assistance
- Not Currently (No active permanent housing plan at this time)
This field reflects the anticipated housing intervention strategy.
5. Veteran Housing Plan Date Created
Definition: The date the permanent housing plan was created with the Veteran.
If no housing plan exists, this may remain blank.
6. Veteran Expected Permanent Housing Date
Definition: The target date the Veteran is expected to be housed.
- This is generally 90 days from initial entry into a Veteran housing services program, but may vary depending on circumstances.
- This is a goal date, not a guaranteed outcome date.
7. Veteran County Location
Select the Veteran’s current county location from the dropdown list.
If the Veteran is outside the listed counties, select “Other” and manually enter the correct location. If a client has moved out of SLO county, the status should be Inactive-Other.
This field ensures accurate regional tracking and coordination.
Best Practices
- Update the assessment immediately when a status change is known.
- Use accurate effective dates whenever possible.
- Do not overwrite prior assessments — create a new one when changes occur.
- Try to ensure consistency between List Status and program enrollment data (e.g., Active–Sheltered should align with shelter enrollment).
- Review during case conferencing to ensure statuses reflect real-time conditions.
