Assessments in Coordinated Entry Program
The Coordinated Entry Program requires users to complete multiple assessments with their clients in order to keep information accurate and current.
Each assessment should be completed at regular intervals → this keeps client information up to date, and helps ensure regular activity to prevent the CE Program from auto-exiting
Locating Assessments
Each assessment should be completed within the active Coordinated Entry Program
Open the client profile → Select Programs → Select the active CE Program → Select Assessments (at the program level)

The assessments will only be available inside the program enrollment

Assessment Types & How Often to Complete Them
- VI-SPDAT → This assessment needs to be completed every 6 months or in the event of a significant change
Select the VI-SPDAT that is appropriate for the household composition and client age INDIVIDUAL VI-SPDAT, FAMILY VI-SPDAT V3, or TAY-VI-SPDAT
- Current Living Situation → This assessment needs to be completed every 90 days or if the client’s living situation changes
- This assessment is important for tracking homeless chronicity
- Status Update Assessment → To be completed every 6 months, or if a change in circumstances needs to be documented
Common Mistakes
Assessments that are completed in a Coordinated Entry program that is no longer active will not populate data to the PHQ

If a VI-SPDAT was completed in an exited program by mistake, use the “Link From Assessments” button to connect the assessment to the correct program . Other types of assessments are not able to be linked.
This needs to be done in the active program


Assessments cannot be recorded at the client level and there will be no option to start a new one (see above for how to locate the right section)

Remember to check the assessment history to see what surveys were last completed and when (See above for guidance on when an assessment is needed)
