Clarity Coordinated Entry Status Update: March 2024
Clarity is LIVE! What about Coordinated Entry?
Coordinated Entry Trainings are available in the HMIS Training Site! There are 3 training modules for Coordinated Entry, including an orientation, Clarity workflow updates and the VI-SPDAT upgrade training. Modules also include required forms for the updated VI-SPDAT 3.0 (Individual and Family versions only, TAY 2.0 is the same assessment). Login to the training site to review the trainings!
What should assessors, case managers and housing navigators be doing now?
- Identify clients to reassess and conduct the VI-SPDAT 3.0! If possible, locate and reassess the most vulnerable clients (highest VISPDAT score) first. Bell Data had a significant number of clients that were inactive or unable to be located. Clients that can not be located for reassessment should be exited from Coordinated Entry. Each assessor should review their clients to determine a continued need for housing.
- Refer clients to the new Permanent Housing Queue in Clarity if they are still in need of a housing intervention! Instructions on this workflow are in the 03.02 training module.
- Continue to enroll and assess new clients in the system. Incorporate diversion into initial interview.
- Attend monthly Coordinated Entry Case Management meetings for workflow tips and updates!
The CE Case Management Meeting is for Coordinated Entry Participating Agencies and Case Managers only.
For the calendar invite to the Coordinated Entry Case Management Meeting (4th Tuesday /Monthly/ 1:30 pm PST, contact lsearles@capslo.org.
What should housing providers be doing now?
- Report current housing openings to CE System Manager for referrals (this is the current process). Do not allow vacancies to sit open!
- Prepare to create vacancies in Clarity (see training opportunities below):
