New HMIS Program Request Form

HMIS Agency Administrators may submit new project request forms via the SLO HMIS Portal. New project requests will be reviewed and the program will be created in Clarity.

• Agency and Project Information
• Bed & Unit Inventory Information (Residential projects only).
• HMIS Funding and Grant Dates
  • HUD and Federal Partner Funding Reference
  • AB-977 Reference
• Project Service Information, if applicable - View existing HMIS Services to create your list.
