Adding a SSVF Referral Event

The Community Action Partnership of San Luis Obispo (CAPSLO) Supportive Services for Veterans and Families (SSVF) program is accepting referrals to their rapid rehousing program through the Coordinated Entry System (CES) in the Homeless Management Information System (HMIS) database.


  1. After locating the client profile, select the active Coordinated Entry program on the right-hand side of the screen.

🛑 Be Sure to confirm the client's Veteran Status is accurate on the Client Profile page


  1. Once on the program page, select the Events tab.

  1. Select “Referral to RRH Project Resource Opening”

  1. Complete the referral form → Select the Location of Crisis Housing or Permanent Housing Referral: CAPSLO SSVF Rapid Rehousing (PRO73) → Referral Result: Can be left blank and then filled out later → Add an event note (optional, but recommended) → Select Submit


Direct Referral to SSVF Project

This is a notification to the Responsible Staff

  1. Start by selecting the Referrals tab from the list of global variables at the top of the screen. You do not need an enrollment for this action.

  1. Select the CAPSLO Agency, select “All” under Program Availability, and hit Search.

  1. Select CAPSLO SSVF Rapid Rehousing project and click Make Referral.

  1. Select Send Referral, and the SSVF team will be alerted that a referral has been submitted. The team will contact the client directly or the client's care team to determine eligibility. Please share any information that would help with this process on the referral's final page.

Note: It is not necessary to exit consumer from Coordinated Entry Enrollment, as the CE Event will auto exit the client once they are enrolled in SSVF and have a housing move in date.
