Use this checklist as a quick reference to confirm that all data entry steps for Coordinated Entry have been completed.

CE Clarity Data Entry Checklist

  1. Does the client have a current ROI for Coordinated Entry?
  2. Does the Client have an Active CE Enrollment?
    1. Complete Enrollment Assessment
      1. Make sure to complete the Diversion questions
    2. Enrollments without the activity described in step 3 "assessments" below will be auto exited from Coordinated Entry at 120 days.

ACTIVITY

  1. Are the following assessments complete?
    1. Crisis Needs / Diversion Assessment (at enrollment only)
    2. Current Living Situation Assessment (multiple CLS assessments will be recorded over time)
    3. Housing Needs Assessment (VI-SPDAT; at least once every 6 months or updated if a significant change occurs for the client. )

Note, per HUD Guidance for Coordinated Entry, record Current Living Situation Assessment anytime the following occurs:

  1. A Project Start is associated with Coordinated Entry; or
  2. A Coordinated Entry Event is created (i.e. Housing Referral); or
  3. The client's living situation changes; or
  4. The current living situation hasn't been recorded for longer than a community-defined length of time (i.e. longer than 90 days). Currently SLO County CoC is using 90 days as the defined length of time.
  1. Has a Diversion Event been added to the client record?
    1. If Diversion needs identified in the Enrollment Assessment have been addressed, add:
      1. Diversion Event
      2. Diversion Result
  2. If needed, Has a Referral to the Permanent Housing Queue been completed?
    1. Toggle on 'SLO Co. CoC Permanent Housing Queue'
    2. Select 'Refer Directly to Community Queue(s)'
    3. Confirm & Submit

A full list of referrals made on behalf of the client can be viewed under the History tab from the Client Profile page.
