Coordinated Entry- Referral to SLO CoC Permanent Housing Queue Queue

Clients must be referred to the Community Queue in Clarity HMIS. The referral to the queue happens after the client has been enrolled in CE and an assessment has been conducted. Follow the steps outlined below to refer your client. For a checklist of all Coordinated Entry data elements, refer to the Coordinated Entry Checklist Article.

  1. After enrolling the client, start the appropriate assessment:
  2. There are 3 VI-SPDAT assessments, choose the appropriate assessment and click START.

  3. Immediately after completing the assessment, the eligibility score page will load. Toggle the SLO County CoC Permanent Housing Queue button and Click REFER DIRECTLY TO THE COMMUNITY QUEUE(S).
  4. The referral is two steps, toggle the Queue and Refer

  5. The referral page will load and the user can add a note to the CE System Manager. This generates a Clarity email to the CE Staff, please include any additional details for the referral in the text box.
  6. THE REFERRAL IS NOT SENT UNTIL YOU CLICK SEND REFERRAL.

    NOTE: Please do not refer clients to Coordinated Entry from the REFERRALS tab. The system will move the user FROM the assessments tab TO the referrals tab, but the referral must be initiated from the assessment eligibility step above.


  1. To confirm if a client is already on the SLO County CoC Permanent Housing Queue, simply check their profile! This message reads, "Client has an active entry on the Community Queue..."
  2. Also, if the red box reads, No Contact Information, Click Add to put in a telephone number, email or alternate contact for the client to ensure they can be reached!
  3. This client is on the queue, but they don't have any contact info on file!
