Coordinated Entry Updates: May 2024

Assessment Review

Status Update Assessment and Annual Assessment have been added to the Coordinated Entry Program Assessments List! Status Update Assessments should be completed at least once every 6 months.

Reminder: Current Living Situation Updates should be completed each time a case worker meets with a client and collects data on the clients living situation to establish the need for homelessness prevention or the need to be further assessed on for the Coordinated Entry Permanent Housing Community Queue.

Available assessments within the CE Program Enrollment.

SLO County Crisis Needs/ Diversion Assessments should be completed on clients where no Housing Needs Assessment (VISPDAT) is conducted due to the nature of services provided. For example, a problem solving conversation leads to the client no longer needing additional emergency services (diversion from homeless services).

In the case of diversion, an Event should also be added to the client record, indicating the outcome (client housed/ re-housed in a safe environment).
