Coordinated Entry Reports - Permanent Housing Queue

8+ not on PHQ Report

Clients with a VI-SPDAT assessment score of 8+ qualify for housing and therefore may want to be. The “8+ not on Q” report generates a list of clients who have an assessment score 8 or higher and are not on the permanent housing queue.

Why CE sends out this report

Clients who have a VI-SPAT 8 or above qualify for housing resources. CE wants to make sure that every client who qualifies for housing has an opportunity to get on the queue as quickly as possible, so that those individual clients can begin increasing their priority and find housing opportunities faster. 

Reviewing the report

Case managers (CM) will review the list of clients with a score 8+ not on Q. If a CM finds their client on the list, the CM will add their client to the Community Queue unless their client does not wish to be added. 

Understanding the fields in the report

From the Case Manager (CM’s) perspective, the data fields are the columns in the report. Below is a table that gives a definition of each of the fields. 


Fields Definition
Clients Client ID A numeric combination that corresponds to an individual client profile in Clarity, however this ID is not searchable on Clarity and is mostly used for backend purposes.
Clients Client Full Name Gives the clients full name in Clarity.
Clients Unique Identifier An alpha-numeric combination that corresponds to an individual client profile in Clarity, this ID IS searchable in clarity and is a good tool for case managers to identify individual client profiles, especially those with duplicate or confusing names.
Clients Assigned Staff Staff member who assessed client.
Client Assessments Assessments Date The date the assessment was given. 
Client Assessments Assessment Score The score the client got on the assessment.
Client Referral Status Is client currently on queue? A field with a yes/no value. “Yes” if the client is currently on the queue, “No” if the client is currently not on the queue.
Client Assessments User Home Agency The home agency of the user who gave the client their assessment. 
Enrollments Enrollment ID This is a numeric ID that correlates to a client’s coordinated prado access center enrollment  page. It is mostly used for internal backend purposes, such as to create links.
